TRICARE
Active duty service members are REQUIRED to enroll in TRICARE Prime and MUST fill out an enrollment application at the Presidio of Monterey Health clinic (DLI).
Family members may choose to enroll by submitting a completed enrollment form at the Tricare Service Center at the Presidio of Monterey Annex at Fort Ord (POMA) Monday thru Friday from 0800-1630. They
will provide additional assistance regarding complete program option, eligibility, benefits, claims and access to care. For 24 hr assistance by phone, call 1 (888) TriWest.
