Degree Approval Form – Due 4 quarters before graduation. This is your “contract” with the chairman and Curriculum Officer to ensure that if you complete your courses as planned, that you will qualify for your degree and P-code.
The appropriate degree checklist and a printout of your matrix from Python should be included with the Degree Approval Form.

Thesis Approval Form – Due at least two quarters before graduation. This formally assigns your thesis advisor. Once your Thesis Approval Form has been approved, thesis slots will be added to your matrix.
